If you are unable to complete the New Patient Paperwork online, you will need to arrive to the office 30 minutes prior to your scheduled appointment time to complete this paperwork in the office.
What To Bring To Your Appointment
Insurance cards and valid Government Issued Photo Identification
Names and contact information for your Primary Care Physician
Medical Records from your healthcare provider if you were referred to our office
Referral and/or Authorization from your insurance if your insurance company requires it
List of all current allergies and medications
We perform a complimentary benefits verification for you before you come into the office. Payment for services such as deductibles, co-insurance, and co-payments are due at the time of service. You are responsible for obtaining any applicable referrals/authorizations for treatment from your insurance company (if applicable) before coming in for your first appointment with us.
